Any applications received by Work Permits (UK) on or after 1 June 2005 will only be accepted if they have been submitted on the most up to date version of the application forms. These will be dated April 2005.

Any previous versions of the application form, obtained prior to 1 April 2005, should be destroyed immediately as these do not include up to date charging information and are therefore no longer valid.
